Hospital Discharge and Assessment Team (HDAT)

The HDAT is now a team of 50 plus staff with a supportive and engaged leadership structure including an Integrated Social Care Manager, two Deputy Social Care Managers, five Social Work Leads, a Social Care Lead and two Care Navigator Co-ordinators. The team consists of a Senior Social Worker, Social Workers, Adult Social Care Practitioners, Care Navigators and an administration team.

Our team is committed to integrated working with health and therapy colleagues. We encourage a ‘Home First’ approach to hospital discharges, ensuring adults are going home with the right support, or if required to a short term bedded environment for recovery and reablement. We have a person-centred ethos and commitment to making a difference in the lives of the adults we work with, aiming to get them back to their   own home and to living independently. The team also operate an admission avoidance advice line, 7 days a week, offering support to Emergency Department colleagues for admission prevention pathways and signposting.

Our team office base is at Shire Hall, Gloucester but works countywide across the following locations and hubs:

  • Gloucestershire Royal Hospital – ED and pathway review hub
  • Cheltenham Fire Station
  • Charlton Lane Mental Health Hospital – Social Work Hub
  • Care Navigators supporting Cheltenham General hospital , Gloucester Royal Hospital , the Complex Care at Home teams and Community Hospitals across Gloucestershire
  • Assessment Bed Unit across Gloucestershire
  • Continuing Healthy Care (CHC) SW Hub

Our locations

Gloucestershire Royal Hospital is based in the city centre of Gloucester, it is within walking distance to the high street where you can find shops and cafes and is a short walk from the train station. Gloucester train station has good rail links to the surrounding areas in Gloucestershire as well as Bristol and London.

Charlton Lane Hospital is based on the outskirts of Cheltenham and not too far from the border of the Cotswolds, an area of Outstanding Natural Beauty. Charlton Lane has free parking available on site.

Shire Hall is in the heart of Gloucester City Centre. There are plenty of cafes and shops in the city, all within walking distance. Shire Hall is also situated opposite Gloucester Cathedral where there are benches and outside seating areas available, where you can take time out during the summer months.

A 10–15-minute walk will take you to Gloucester Quays, an outlet shopping centre with a mix of high street and designer labels as well as a great selection of restaurants. There is free parking available to those working in the Gloucester Locality, which is only a short walk from Shire Hall. There is also a limited amount of duty spaces available to workers on the Referral Centre and a smaller onsite car park where you can park for 3 hours at a time, which is ideal for days when you have planned visits or external meetings.
