Graduated Pathway - Information & FAQ's
The Graduated Pathway is Gloucestershire’s response to ensuring Early Help is available to all children, young people and their families with additional needs, whether these are educational, social or emotional needs or a disability. Early Help Training
Early Help training modules are available to help you understand how to complete assessments and plans as part of the graduated pathway. Please click the link below for more information about this training as well as other multi-agency training opportunities.
Registering Plans and Assessments
For any child or young person, and their family, who are receiving support through the Early Help Graduated Pathway, please complete an Early Help Notification Form when there is:
- An existing My Plan or Assessment and My Plan+ that the Early Help teams have not been previously notified of.
- A new My Plan or Assessment and My Plan+.
- A change of Lead Practitioner.
- Closure of a My Plan or My Plan+.
