You must provide evidence

When applying, you should provide evidence showcasing how you have been financially effected from COVID-19. Please refer to our advice below on what supporting evidence you can supply for your application.

For a successful application, you will need to explain in detail how COVID-19 has financially affected you under the section ‘Please tell us here what you need and why COVID-19 has created this need:’.

Once you’ve completed your application, please send all evidence to: Childrensfund@gloucestershire.gov.uk

What evidence can I supply?

Below are some examples of evidence you can supply. Please bear in mind this is not an exhaustive list, so other forms evidence may be accepted.

Furlough

If you are currently on furlough the evidence needs to show:

  • Employers contact details
  • Dates of furlough period
  • Pay level during Furlough period

Examples of evidence you could provide:

  • Furlough letter from employer
  • Email from employer confirming furlough status
  • Payslips showing furlough payment

Loss of income due to COVID

Examples of evidence you could provide:

  • Old and recent payslip
  • Email/letter from employer confirming loss of hours or redundancy
  • P45 letter

Increase in living costs due to COVID

Examples of evidence you could provide:

  • Recent bill (Utilities, rent, council tax etc.)
  • Picture of gas and electric metre
  • Most recent universal credit statement

Having to isolate/shield due to COVID 19

  • Most recent shielding letter
  • Screen shot of NHS notification informing individual of positive test
  • Letter from NHS informing individual as being a contact of someone who tested positive for COVID 19
  • Letter from school informing children to isolate due to COVID positive test in year group
