The Gloucestershire 5 Step Approach: Personalised Care Planning for Behaviours that Challenge in Dementia

The Gloucestershire 5 Step Approach is an initiative to develop a person centred care planning approach across Gloucestershire for people with dementia who experience behaviours that challenge.

A person with dementia will sometimes behave in a way that others find difficult or challenging to manage. Behaviour changes can be a result of the progression of dementia but it may also be a response to emotional stress, physical illness, or environmental changes. These behaviours may cause distress to both the person with dementia and to others including carers, who will need help and support to relieve the distress and develop a plan to improve quality of life.

The pack included here describes a process approach to developing a personalised care plan for the person with dementia and their carers, and has all the resources needed to put the approach into practice
