Long Term Conditions
Long term conditions have been identified as a priority by the Gloucestershire Integrated Care System (ICS).
Long term conditions include any ongoing, long term or recurring condition requiring constant care.
Examples of long term conditions include diabetes, heart disease and chronic obstructive pulmonary disease. Chronic conditions and their effective management is widely recognised to be one of the greatest challenges facing the NHS and Social Care. They can affect many parts of a person’s life, from their ability to work and have relationships to housing and education opportunities.
