TOP Form guidance
As a health or social care practitioner you are now able to use the Telecare Online Prescription (TOP) Form to request some core items of Telecare equipment following your own assessment.
(TOP TIP! Some people are finding that the TOP Form is crashing mid-way through completion and work is lost.
You can avoid this risk by signing up for 'My Account' BEFORE you complete the form. (The link is top right of the form.) This will save as you go.
If a form is partially entered but you are forced to leave, you can return to it later to complete)
The TOP form can be used by any health or social care practitioner regardless of whether you're GHC or GCC staff. You don't need to be a trained Telecare Level Two assessor, and you don't need access to the Liquidlogic Adults System (LAS)
The TOP Form allows you to request the following key items;
- Verbal prompting device
- Orientation clock
- Floor mat and pager
- Lifeline with linked smoke and heat detectors
- Vibrating watch
- Wrist-worn falls detector and linked lifeline unit
It still remains your professional responsibility to complete your own assessment, and complete the form as fully as you can. The Specialist Telecare team will not assess the person on your behalf; this form is purely an equipment request form.
