My Plans

When you are looked after plans must be made for all the important things in your life.
The main plan is called your Care Plan which is written information that says how you should be cared for and describes your future plans.
You will have other plans too for things like where you are living and your education. All of the plans should link together to make sure you are getting all the help and support you need while you are looked after.
Care Plan
Your care plan will include:
- why you are looked after, what needs to happen for you to go home or if you can’t go home what the long term or Permanency plan is
- your wishes and feelings
- contact with family and friends
- your health
- education and training - you will have a Personal Education Plan (PEP) which is another plan
- any extra help or support you need to manage your feelings
- how you wish to follow your religion, beliefs and culture
- hobbies and interests.
Placement Plan
This is part of your Care Plan but will give more practical details about how the people you live with should be supporting you.
It will include:
- your living arrangements
- getting to school
- who you will be able to see, or not see
- how much pocket money you’ll get
- what time you have to be in
- your travel arrangements.
Pathway Plans
When you reach about 15 1/2 yrs old we’ll start looking to the future with you, what you want to do and what you might need help with.
There are lots of options when you get to 18 years old and as a responsible Corporate Parent we want to make sure you are able to manage your money, live on your own and be independent. We do this through having Pathway Plans.
