When you join the LGPS
There are two essential forms, known as the starter pack, that you will need to complete immediately. Your employer should supply these to you.
Pension starter and service history form
Complete this form to provide us with your personal details, and to request transfers of previous schemes.
If you are a member and have not yet completed this form, please complete it and send it to us. For us to action any aggregation elections or to investigate any transfers, it needs to be completed and sent to us.
Expression of wish form for death grants
Complete this form to express your wish of who you would like to receive any lump sum grant payable from the Fund, in the event of your death. We strongly encourage you to complete this information. (Bear in mind that completion of this form outlines your preference, yet the final decision on who should receive any death grant rests with the Fund.)

Adjusting your contributions
50:50 contribution flexibility form
Complete this form to opt for the 50:50 section or to return to the main section of the scheme.
Please do not attempt to upload this form onto the Engage portal.
Going into retirement
Active members
You only need to complete this form immediately after leaving your employment, if you wish to claim retirement benefits from your active membership. Your employer should provide this form to you at the appropriate time.
Deferred members
If you're no longer employed and have deferred your pension, complete this form when you're ready to claim your deferred retirement benefits.
If you leave the LGPS
Complete this form if you have left your employment and you wish to do one of the following:

- Receive a refund (subject to eligibility).
- You are aged 55 or over and wish to investigate receiving your pension or you wish to transfer out your pension rights to another scheme.
Opting in or out of the LGPS
You need to send the following Opt in and Opt out forms to your payroll provider.
If are eligible to join the LGPS but not currently a member because you are either a casual employee or have previously opted out, complete this form and send it to your payroll provider.

If you wish to opt out of the pension scheme, you need to complete this form and send it to your payroll provider. Opting out is an important decision as you will lose out on valuable benefits - therefore we recommend you carefully read the form before taking this course of action.
