Applications to the DoLS Team should be made using the online form located in our Adult Social Care Professional Portal.
Our guidance document provides instructions about how to register to use the portal.

Managing authorities have a responsibility to ensure that the relevant person's family, friends and carers are informed that an application for a Deprivation of Liberty Authorisation has been made, unless it is impractical or impossible to do so.
Relevant Person’s Representative (RPR) Information Booklet
RPR’s are appointed to support a person who is deprived of their liberty under the Mental Capacity Act 2005 Deprivation of Liberty Safeguards (DoLS). The information provided in this booklet will help RPR’s to understand the DoLS process and the support available to them.
