I'm a professional and want to make a referral

We want to simplify the way you can refer into the Technology Enabled Care team. We've therefore decided to remove the Telecare Online Prescription (TOP) form as an assessment tool.

How to make a Technology Enabled Care assessment

  • If you are an external professional, please continue to contact the Adult Social Care Helpdesk on 01452 426868 to make a referral.
  • If you work for GCC, you should refer into the Technology Enabled Care team using our internal case management system (LAS).

We will continue to prioritise those who require an assessment to support with hospital discharge, alongside completing our non-urgent assessments.
