We work with younger and older adults of all abilities and needs.Ā
We have moved to the three conversation approach, a model that re-emphasises focus on people, their strengths and the networks and community around them
Our team is currently split into four hubs which all have a lead Social Worker or Social Care Assessor and around six other workers in each hub. These smaller teams allow for better opportunities for reflective working and peer supervision.
We also work a team across two community hospitals (one in Cinderford and one in Lydney) and we have a worker from the team work from each hospital.
