The Hospital Discharge & Assessment Team (HDAT) is now a team of 70 plus staff with a supportive and engaged leadership structure including an Integrated Social Care Manager, an Operational Lead for Adult Social Care and Hospitals, Deputy Social Care Managers, Social Work Leads, Social Care Leads and Care Navigator Co-ordinators.
The team consists of Senior Social Worker, Social Workers, Adult Social Care Practitioners, Care Navigators, and an administration team.
Our team is committed to integrated working with health and therapy colleagues to implement the discharge to assess model and home first approach. We do this through working with adults upon discharge who may require short-term care, rehabilitation and reablement, where needed, and then assessing people’s longer-term needs for care and support once they’ve reached a point of optimal recovery. This may be in adult’s homes or Pathway 2 and 3 beds to support the transition from hospital to home. We have a person-centred ethos and commitment to making a difference in the lives of the adults we work with, aiming to get them back to their own home and to living independently.
"My day-to-day work is to support the team, to prevent adults who are medically unstable for discharge to be admitted to hospital and support with safeguarding.
