The Hospital Discharge & Assessment Teams work across the following locations and hubs:
- Integrated Flow hub – Gloucestershire Royal Hospital
- Pathway 1 Progression Hub – Working countywide
- Pathway 2 and 3 Progression Hub - Working countywide
- Charlton Lane Social Work Hub – Cheltenham
- Care Navigator Hub - Working countywide covering Acute Hospitals, the Complex Care at Home teams, Pathway 2 and 3 beds across Gloucestershire including Community Hospitals
- Continuing Healthcare (CHC) / Social Work Hub – Working countywide
- Leadership and Administration Hub – Shire Hall Gloucester

Gloucestershire Royal Hospital is based in the city centre of Gloucester, it is within walking distance to the high street where you can find shops and cafes and is a short walk from the train station.
Charlton Lane Hospital is based on the outskirts of Cheltenham and not too far from the border of the Cotswolds, an area of Outstanding Natural Beauty. Charlton Lane has free parking available on site.
Shire Hall is in the heart of Gloucester City Centre. There are plenty of cafes and shops in the city, all within walking distance. Shire Hall is also situated opposite Gloucester Cathedral where there are benches and outside seating areas available, where you can take time out during the summer months.
