Service Coordinators (Grade 6)
The Day Services Coordinator works as part of a team to facilitate and coordinate meaningful opportunities for adults with social care needs, including physical disabilities, learning disabilities and dementia, which includes daytime activities, both on site and in the community. Their roles include coordinating and supporting the team with the day-to-day running of the service, whilst also supporting individuals with all aspects of their day-to-day living needs, including coordinating care plans, maintaining social and physical activities, supporting with medication needs, personal care, mobility, and mealtimes.
The Respite Services Coordinator works as part of a team to facilitate and coordinate meaningful opportunities for adults with social care needs, including learning and physical disabilities within our 24-hour regulated respite services, both on site and in the community. Their roles include coordinating and supporting the team with the day-to-day running of the service, whilst also supporting individuals with all aspects of their day-to-day living needs, including social and physical activities, personal care, mobility, and mealtimes.
The Rehabilitation Services Coordinator works as part of a team to coordinate rehabilitation and care programs that promote and develop independence and reablement for adults discharged from hospital or admitted from the community. Their roles include coordinating and supporting the team with the running of our 24-hour regulated service, whilst also supporting individuals with all aspects of their day-to-day living needs, including personal care, mobility, and mealtimes, by following therapy-led rehab goals.
The Rehabilitation Services Night Coordinator works as part of a team during evenings, overnight and early mornings, to coordinate rehabilitation and care programs that promote and develop independence and reablement for adults discharged from hospital or admitted from the community. Their roles include coordinating and supporting the team with the running of our 24-hour regulated service, whilst also supporting individuals with all aspects of their night-time needs, including personal care and mobility, by following therapy-led rehab goals.
