Activities Coordinator (Grade 5)

The Rehabilitation Services Activities Coordinator works as part of a team to promote the independence and reablement for adults discharged from hospital or admitted from the community. They work with our Therapy team to produce and coordinate stimulating and engaging activities, to include both one-to-one and group sessions, in line with rehabilitation and care programs that promote and develop independence within our 24-hour regulated service and to realise individuals goals.

The Rehabilitation Services Activities Coordinator motivates, encourages, and participates in activities with individuals to support in maintaining their physical, emotional, and social wellbeing, dignity, high quality of care, and to meet rehabilitation goals of those individuals attending the service.
