Care Navigator Coordinator (Grade 8)

The Care Navigator Coordinator is a countywide management position.

As a Care Navigator Coordinator you will supervise a countywide team of Care Navigators. This will involve delivering supervision, appraisals, monitoring training and development as well as overseeing the day to day running of the team.

Care Navigators work with people who are deemed as not eligible for social care needs but have been identified by health/social care colleagues as needing ongoing support when discharged from hospital in line with the Care Act 2014 advice and information.

As a Care Navigator Coordinator you will work closely with health professionals, patients and their carers, and family members. You will coordinator between other services, organisations and voluntary sector partners.
