Transitions Manager (Grade 10)

The Transitions Team Manager works within our learning disabilities team, taking on countywide work.

The Transitions Team Manager manages a hub (team) of social care practitioners, taking accountability for the quality of social work practice. Managing this hub, the team manager takes a lead role in embedding the three conversation model, encouraging practitioners to:

• listen to people and connect them to appropriate community resources
• support people in crisis to regain control of their lives
• lead the life they choose in line with the Care Act

The Transitions Manager leads, motivates and manages a team of qualified social workers and unqualified staff.
