How do I make a disclosure request?

Organisations wishing to request disclosure of personal information held by us on an ad hoc basis must complete our online form. You need to ensure that your request includes:

• A countersignature from a senior officer within your organisation.
• Requests for disclosure from the Police need to be countersigned by an officer one rank above the requesting officer or higher.
• For other organisations this needs to be a team manager or the equivalent.
• Details of any legislative requirement to provide the information to you.
• What legislation or legal gateway applies to the requested access; and
• An explanation as to why the information is necessary to meet that obligation or legal gateway.

If you do not complete the form fully or provide all the information necessary for us to consider your request it is likely to delay our response.
