What do I need to make a request?

It helps if you provide as much information as you can to help us locate the data such as previous addresses, names of social workers/contacts within Gloucestershire County Council, but you must provide

• Your name.
• Your address.
• Your date of birth.
• Details of the information you are requesting to have erased or restricted
• Your specific reasons as to why you want your personal data erased or its use restricted. These reasons should be based upon your particular situation.
• Proof of identification. We accept:
• a copy of your birth certificate or
• a copy of your passport or
• a copy of your driving licence or
• verification by a council officer who has been working with you or if you cannot provide any of the above,
• a copy of 2 different utility bills and/or bank statements from within the past 3 months
• Proof of change of name (if applicable).
• If you are acting on behalf of another individual, we will need their proof of identity and proof of written consent for you to act on their behalf or appropriate power of attorney.
• If you would like our correspondence with you to be by post, we will also need verification of your address, this could be a utility bill (dated within 3 months of your request) or council tax bill.
