Introduction from Keith Vardy, Head of Adult Social Care Operations
Hi, my name is Keith Vardy and I am the Head of Adult Social Care Operations.
My responsibilities include all locality adult teams (Chelt/Glos/Stroud/Tewks/Cots/Forest), a countywide Hospital discharge team, and a countywide Learning Disability Team. In addition, and affiliated to these are an Enablement Team (with 6 Drop In’s), a Sensory Team, and a Positive Behaviour Support Team.
I also have oversight of 3 CQC regulated services (Wheatridge Court, Great Western Court and Gloucestershire Shared Lives).
Sitting within the Learning Disability team is the Transition Team. This small team has a Team Manager, a Social Worker and 5 Assessors. The Team coordinates all work (from an adult perspective) with young people who are likely to need ASC support post 18. The team work alongside colleagues in Children’s services and education to identify these individuals from around the age of 16, if not before.
All Adult teams follow a strength based model of Assessment that we call ‘Make the Difference’. We try to get to know those that are referred to us - our initial contact and ongoing engagement is focussed on helping, and understanding what and who is important to the individual in the life they wish to lead.
We will have a spotlight article on the Transition Team in the Families In Partnership February issue.
