Additional tools to inform My Plan/My Plan Plus

We hope you find these tools useful to support you in direct work with children/young people and families.

These are the tools we recommend you use to inform My Plan and My Plan Plus, however you may be more familiar with using different tools which can equally facilitate your direct work which you may prefer to use.


My Profile

My Profile helps a child or young person share personal information to help those involved with them understand their motivations, preferences, goals, and self-image. The design of this template may vary depending on a child or young person’s interests and preferences. As My Profile shares personal information, consideration needs to be given to who this is shared with. A child or young person should be informed how the information will be used, for example: helping to plan for transition to another year group, school, health provision or care setting.

Most children or young people who have a My Profile will not have been identified as having additional needs or requiring additional agency support.


My journey

My Journey is a tool to aid direct work with children and young people and should be used flexibly dependant on individuals needs and preferences to gain their voice in assessment and planning. The tool enables you to work with the child/young person to understand important things about them and also to support conversations so that they understand what needs to happen and why. This tool can be used before the My Plan or My Plan Plus is created to help shape and get their views, or it could be used to prepare for a review of a My Plan or My Plan Plus to look at what has changed for the child/young person or family.


My Progress Charts

The main purpose of the My Progress Charts is to help measure improvements or distance travelled over time and provide a visual representation to the child / young person / parent / carer. You can use the My Progress Charts with a My Plan or My My Plan Plus and as part of the initial engagement process as well as at review points.
