My Plan Plus*

My Plan Plus is undertaken when the child's needs are more complex and require a multi-agency assessment of need to understand the range, depth or significance of those needs which may require a more intensive response. My Plan Plus must be informed by a holistic assessment; The recommended format for this is the My Assessment, however an alternative holistic, whole family assessment may be used to inform the My Plan Plus. All agencies who know or who are involved with the child / young person and family should contribute to the holistic, whole family assessment of need, leading to a SMART (Specific, Measurable, Achievable, Realistic and Time bound), outcome focussed My Plan Plus that is supported by appropriate agencies and reviewed on a 6 to 12 weekly cycle.

*My Plan Plus identifies that the plan is informed by a holistic assessment and may be created to meet needs identified in a single assessment that results in no further Social Care involvement or an Education, Health and Care (EHC) assessment.
