Outcomes of a single assessment undertaken by a social worker

  • The child and family needs a statutory intervention and a social worker is allocated and a plan is developed to meet the need – this might be a Child in Need Plan or Child Protection Plan.
  • In rare occasions, emergency protective action may be taken and the child becomes Looked After either in the short term or longer term.
  • No role for Children’s Social Care.  The Social Worker who conducted the Single Assessment may have identified needs that could be met through an Early Help response and will gain consent from the family. These needs may be met:
    - Through universal services or a single agency (Levels 1 and 2); or
    - A multi-agency Early Help My Plan+ (Level 3) informed by needs identified through the Single Assessment. The Social Worker can refer the family to GCC internal or commissioned family support work services using the ‘step down’ process outlined below.
