What should happen if the level of need decreases?
- Discuss with the child and their family
- No further additional support required
What should happen If the level of need increases?
- Discuss with the child and their family
- Contact Gloucestershire County Council’s Early Help Advice, Guidance and Support service and ask if a My Plan has been registered for the child
- If a My Plan has been registered, contact the Lead Practitioner to discuss your concerns
- If there is no Graduated Pathway Plan registered create a My Plan and if a multi-agency response is required to meet the needs identified then arrange a Team Around the Family (TAF) meeting inviting relevant agencies
