Early Help - Family Support Work (FSW)
Level two
GCC Targeted FSW may step down assessments to community based lead practitioners as the FSW involvement is due to close. A handover TAF takes place and a new Lead Practitioner is identified.
Level three
- GCC targeted FSW is often mobilised following a step down from statutory children’s social care services and an EH plan is devised informed from the Social Care assessment.
- GCC targeted FSW work mostly at level 3 and use the graduated pathway (Early Help assessment and plan).
- While FSW are involved they will be the lead practitioner. The My Assessment and My Plan+ tools are built into the Early Help module care recording system.
Level four
Once a child/ young person ‘steps up’ from GCC Targeted FSW to a level 4 statutory children’s’ social care service the my plan/plus are paused and replaced by a statutory assessment.
