Year 11 transition support
Level two
- Vulnerable learners at risk of not successfully transitioning to Post-16 education are brought to transition panel. The panel links the feeder school with the receiving college and interventions are implemented, where appropriate.
- The panel signposts to alternative provision where mainstream college may not be suitable and facilitates early intervention from agencies, including Transition Chat, if the young person is unlikely to progress to post-16 education.
Level three
- Transition Chat is available to young people in Year 11 who are at high risk of becoming NEET following their secondary education. * Referrals principally come from the transition panels but young people can self-refer. These young people will have a variety of barriers or challenges stopping them progressing to the next step in education.
- They are assigned a Youth Worker who starts work with them whilst they are in Y11 and continues to support through the summer break and into post-16 education until around October/November to ensure they are settled.
- Support offered can deliver against outcomes highlighted in a My Plan Plus, and relationships formed will enable more robust My Assessments to be written that contain the views and lived experiences of Children/Young People.
