Adult Long Term Services: Localities, Learning Disabilities and Enablement Teams
Adult Long Term Services includes our social care assessment and support planning teams in our locality offices as well as our countywide Learning Disabilities Team and our Enablement service. Our teams deliver strengths-based and outcome-focused practice through our high-quality social work service.
Our teams have a supportive management structure including an Integrated Social Care Manager, Deputy Social Care Managers, Social Work Leads and Social Care Leads.
Our frontline practitioners include qualified Senior Social Workers, Social Workers, Adult Social Care Practitioners and Enablement Coordinators. Each team is supported by an effective administration team which includes a Team Leader, Senior Team Administrators and Team Administrators.
