Adult Short Term Services: Day, Respite, Rehabilitation, and Hospital Teams

Adult Short Term Services includes our Inhouse services (Day, Respite and Rehabilitation Teams), and our integrated hospital and out of hospital pathway teams. The service ensures the operational delivery of short-term assessments and support planning as well as the delivery of short-term care provision.

Roles in our Inhouse Services include Registered Managers, Team Leaders, Support Workers and Coordinators. 

Roles in our integrated hospital and out of hospital pathway teams include social care and social work management and leadership, Social Workers, Adult Social Care Practitioners, Care Navigators, and administrative business support roles.
