Preparing for adulthood

Moving from children’s to adult services can feel like a big step for young people with additional needs and for their families. That is why Preparing for Adulthood is an important part of our preventative approach. We want planning to start earlier, feel clearer, and focus on what matters most to each young person.

We are strengthening this by starting planning from age 14, working more closely across education, health, children’s services and adult social care, and focusing on four key areas: independent living, good health, employment and being part of the community. Our aim is to make sure support is joined up and tailored to each young person’s needs, strengths and goals.

Feedback from young people and families is helping shape this work. They have told us how important it is to be listened to, have clear communication, and avoid the “cliff edge” that can happen during transition. We are using this learning to guide the next phase of improvement.
