- Timely Assessments and Reviews: With additional staff focusing solely on Care Act reviews, reviews completed within 12 months has improved by 10% in one year. This ensures that individuals receive timely support and services tailored to their needs.
- Personalised Support: The focus on Care Act reviews allows for more personalized and timely reviews, ensuring that care plans are tailored to the specific needs and preferences of individuals. This has led to more effective and person-centered care.
