What personal data do we use?

We will collect your personal data in a variety of ways, whether this is via our online or paper forms, from a partner organisation as a referral or from you directly during conversations you have with our services either via telephone, email, in writing or face to face, to be able to deliver the services we provide.
Your personal data that we collect, use and share includes:
- Name and contact details
- Full address
- NHS number
- National Insurance number
- Geographic codes such as postcodes for the analysis of health inequalities
- Date of birth
- Family and next of kin/interested parties details
- Online identifiers such as IP Addresses or cookies
- Lifestyle and social circumstances
- Financial information
- Employment and educational details (including qualifications)
- Residential and right to work status
- Details of any State Benefit receipts
- Housing or social care needs
- Visual images, such as photographs or CCTV recordings
- Location data
- Information held in case files
- Any required additional supporting information including financial information about the organisation (required for Public Health Grants)
We also collect what is known as sensitive personal information (or special categories of data). This is often information you would not want widely known and is very personal to you, types of sensitive data can include:
- Criminal conviction data (including DBS checks)
- Racial or ethnic origin
- Political opinions
- Religious or philosophical beliefs

- Genetic and biometric data
- Mental or physical health details (including clinical records, care plans, social care assessments and substance misuse)
