Disability-related concessionary bus pass (applications and renewals)

Bus Pass Scam warning

Some pass holders have made us aware of a social media video claiming that existing card holders must register their cards by the 20th November 2025 to allow their continued use.

This information is false and we believe this may be a scam to collect people's information or obtain money under false pretense.  Please do not follow any links or ring any numbers connected with any such messages.  Please do not share your personal information with anyone associated with these posts.

You are eligible for a disability-related pass if you are over the age of five years and have one of the following disabilities:

  • Are blind or partially sighted
  • Are profoundly or severely deaf
  • Are without speech
  • Have a disability, or have suffered an injury, which has a substantial and long-term effect on your ability to walk
  • Don’t have arms or have long-term loss of the use of both arms
  • Have a learning disability
  • Your driving licence has been refused under section 92 of the Road Traffic Act 1988 (physical fitness).  You won’t be eligible if your driving licence refusal is the result of persistent misuse of drugs or alcohol
  • Are unable to drive due to medical grounds
  • Have a valid blue badge
  • Have 8 points or more for ‘moving around’ or communicating on Personal Independence Payments (PIP).
  • Disability Living Allowance – mobility component
  • Armed Forces compensation Scheme Award or War Pensioners mobility supplement.

If you are unable to travel alone due to your disability you may be eligible for a companion bus pass (Click here for a companion C1 form).   This will allow you to be accompanied by one extra person over the16 years of age, free of charge as long as the companion is travelling with the pass holder.

The most straightforward method is to apply for your pass is online.  You will need to provide evidence of eligibility electronically for online applications.  Evidence forms can be downloaded from the links on the right of this page or you can request copies by emailing BusPasses2@gloucestershire.gov.uk.

For customers who are unable to use the online system, you can contact a participating Gloucestershire library to make an appointment to process your disability-related bus pass application.  The libraries will take your photograph when you attend your appointment but you must bring your documentation evidencing your disability and proof of your address.

You will need to provide the following evidence to complete an application:

Photograph – this must be a recent, colour, full face without a hat or sunglasses (unless you are partially sighted or blind).

For online applications you will need a digital passport sized photograph which needs to be in either pdf or jpg format.  If you are making your application through a library your photograph will be taken during the application.

Proof of age - can be one of the following:

  • Birth certificate
  • Driving Licence
  • Medical prescription or optician’s prescription 
  • Passport

Proof of Address - can be one of the following but must show your name and address:

  • Benefit letter
  • Bank statement
  • Council tax bil
  • Driving Licence
  • Medical prescription or optician’s prescription
  • NHS medical card
  • Utility bill

Blind or partially sighted

Please provide one of the following types of evidence:

·         Certificate of visual impairment (CVI) completed by an ophthalmologist

·         Evidence of registration with the local authority as being severely sight impaired (blind) or sight impaired (partially sighted – eg registration card)

·         Veterans – The ‘Reasons for Decision letter under the Armed Forces Compensation Scheme (AFCS).

Profoundly or severely deaf

Please provide one of the following:

·         Veterans – the ‘reasons for Decision’ letter under the Armed Forces Compensation Scheme (AFCS)

·         A D2 form signed by your doctor or consultant.  Click here to download a D2 form

Without speech

Please provide one of the following:

·         Personal Independence Payments (PIP) statement of entitlement which shows 8 or more points for ‘Communicating’

·         For a child, a Statement of Special Educational Needs or Education, Health and Care Plan (EHCP) which states that the applicant is non-verbal.

·         A D8 form signed by your doctor or consultant.  Click here to download a D8 form

Severe walking disability

Please provide one of the following types of evidence:

·         A Disability Living Allowance (DLA) letter stating you are on the higher rate mobility component dated in the last year.

·         Personal Independence Payment (PIP) document with either 8 points or more for ‘moving around’ or ‘communicating’ only.  You will need to send the whole document that includes the scores you receive for each criteria.  If you do not qualify under PIP, then it is advisable to choose a different option i.e signed D1 form

·         Blue Badge showing an end date of more than 3 months– if you have a valid blue badge, you will need to put your number on the application form

·         Veterans – proof you are receiving War Pensioners Mobility Supplement, or ‘Reasons for Decision’ letter under the Armed Forces Compensation Scheme (AFCS).

·         A D1 form signed by your doctor or consultant.  Click here to download a D1 form

 NB – if you do not qualify under ‘moving around’ or ‘communicating’, you will need to get a D1 form signed by your doctor instead.

Without arms or have long-term loss of both arms

Please provide one of the following:

·         Veterans – the ‘Reasons for Decision’ letter under the Armed Forces Compensation Scheme (AFCS)

Learning Disability

Please provide one of the following :

·         Information which confirms you have significant impairment of your intelligence i.e. IQ less than 80 AND significant impairment of your social functioning

·         Education Health and Care (ECH) Plan

·         Statement of Special Educational Needs

·         8 points or more for ‘communicating on your Personal Independence Payment (PIP) dated in the last year.

Your driving licence has been refused by the DVLA (Driving Licence Vehicle Authority) on medical grounds

Please provide one of the following types of evidence:

·         A letter from the DVLA dated in the last year stating that your licence has been revoked due to medical issues.

·         A D6 form signed by your doctor, consultant or epilepsy nurse. Click here to download a D6 form

NB - You won’t be eligible if your driving licence was refused as the result of persistent misuse of drugs or alcohol

Unable to Drive due to medical grounds

Please provide one of the following types of evidence:

·         A letter from the DVLA in the last year stating that your licence has been revoked due to medical issues

·         Have a valid blue badge – you will need to write your blue badge number in the relevant section

·         PIP (personal independence Payment) and have 8 points or more for ‘moving around’ or communicating only . You will need to provide evidence of your PIP document dated in the last year, including the scores you receive for each criteria

If you have a disability related bus pass because you suffer from:

·         a severe walking disability  OR

·         because you have had your driving licence refused by the DVLA/are unable to drive due to medical grounds.

then you will need to re-apply and should receive a letter inviting you to re-apply a month before your bus pass expires. 

If you have not received your new bus pass by the beginning of the month it expires, you can renew your disability-related bus pass online.

There is charge of £10 for a replacement of a lost or damaged pass.  A stolen bus pass can be replaced free of charge if a Police crime reference number is provided.

Please note that once a replacement is requested your old card will be cancelled and will not work.  The replacement card will take 5-7 working days to reach you by post.
