Care Navigator (Grade 7)

As a Care Navigator you will primarily be based in hospital settings.

Care Navigators support and advise patients, their families and carers, who have been identified as self funding, or do not wish to divulge their financial information to the Local Authority with sourcing and determining care/support. You will be supporting to ensure the safe and timely discharge of these patients, through helping them navigate the social care system.

The Care Navigator works extremely closely with providers of services, including voluntary and partner organisations to find creative solutions to meet peoples care or support needs and to improve co-ordination and access to support services.
