Support Workers (Grade 4)

The Day Services Support Worker works as part of a team to facilitate meaningful opportunities for adults with social care needs, including physical disabilities, learning disabilities and dementia. They support and facilitate daytime activities, both on site and in the community. Their roles include supporting individuals with all aspects of their day-to-day living needs, including social and physical activities, personal care, mobility, and mealtimes. 

The Respite Services Support Worker works as part of a team to facilitate meaningful opportunities for adults with social care needs, including learning and physical disabilities. They support and facilitate activities within our 24-hour regulated respite services, both on site and in the community. Their roles include supporting individuals with all aspects of their day-to-day living needs, including social and physical activities, personal care, mobility, and mealtimes.

The Respite Services Night Support Worker works with adults with social care needs, including learning and physical disabilities, during evenings, overnight and early mornings, with the support of a senior member of the team. They work across our 24-hour regulated respite services, supporting individuals with all aspects of their night-time care and support needs, including relevant social and physical activities, personal care, mobility, and mealtimes

The Rehabilitation Services Support Worker works as part of a team to promote the independence and reablement for adults discharged from hospital or admitted from the community. They support and facilitate a range of rehabilitation and care programs that promote and develop independence within our 24-hour regulated service. They support individuals with all aspects of their day-to-day living needs, including personal care, mobility, and mealtimes, by following therapy-led rehab goals.

The Rehabilitation Services Night Support Worker works as part of a team during evenings, overnight and early mornings, to promote the independence and reablement for adults discharged from hospital or admitted from the community. They support and facilitate a range of rehabilitation and care programs that promote and develop independence within our 24-hour regulated service. They support individuals with all aspects of their night-time needs, including personal care and mobility, by following therapy-led rehab goals.
