23.    Recording
 
It is essential that staff must have the signed consent of the individual or their representative. The procedures to be followed with regards to specific tasks must be retained on the individual’s file and whenever a task is performed it must be recorded on the file. All recording will be in accordance with the current GCC File Recording Policy and in line with the Disclosure of Service User Personal Information to Third Parties policy.
