Declaration of Acceptance and Participation Form

This form is to be completed by an appropriate representative from any organisation wishing to sign up to the GISPA. By signing, you are confirming that the organisation is able to meet the baseline requirements set out in the GISPA.

A signed Declaration of Acceptance and Participation Form can be emailed to the Information Management Service at DPO@gloucestershire.gov.uk.
